The most likely scenario is that the clock operator gets interested in the game and forgets either to start or stop the clock at a crucial point in the game.

Here is a new twist that happened to me. Arriving at a game site, we found several people working on the scoreboard. When the clock operator reported, he informed the crew that the clock was new. That sounded really good because the likelihood of its failing should have been low.
